Overview of Intake Forms in Allia

Allia helps you streamline your intakes with customizable intake forms that help you gather the information you need to know and begin building an alliance with your clients from the time they sign up.

These forms feed directly into notes, treatment plans, and your client dashboard with no retyping required

All forms are mobile-friendly for easy client access.

Creating and Customizing Forms

To access the Intake Form feature, navigate to the “Management” page in the Allia platform. Then click on the “Intake” tabTo add questions to the form, click the “Add Question” button.

You can select from the following question types:

Questions are optional by default. Turn the “Required” toggle on to make a question mandatory

Delete unwanted questions by clicking the trashcan icon.

You can return to this page at any time to edit your intake form.

Click “Save Form” to save any changes when you are done.

Obtaining and Reviewing Intake Information

Your clients will complete the intake form the first time they use the patient app (part of the onboarding process)

You can review your client’s answers to intake questions by clicking the “intake” button in their dashboard page.
