Introduction

Welcome to zant. mobile app (the “Site”), which is owned and operated by zant. Inc. (the “Company”), a Delaware corporation (the “Company”, “we”, “us”, “our” interchangeably).

By visiting the Site and accessing the information, resources, Services (as defined below), Platform (as defined below), products, and tools we provide to Users (as defined below) (the “Resources”), we provide, you understand and agree to accept and adhere to the following terms and conditions as stated in this Privacy Policy (hereafter referred to as this “Policy”). Capitalized terms used but not otherwise defined shall have the respective meanings ascribed thereto in the Company’s Terms of Service located at https://www.zant.app/terms-of-use (the “User Agreement”).

This Policy applies to the following individuals:

• All visitors to the Site (“Site Visitors”);
• Anyone who downloads or uses (“Application Users”) the Company’s web or app-based software Application (the “Application”, and together with the “Site”, the “Platform”);
• Anyone who enrolls for the Services of the Company as a patient (“Client Users”) or a service provider (“Providing Users”, and together with Client Users, “Subscribers”).

The Company provides the Platform to connect Client Users with Providing Users for the exchange of products and services offered through the Platform (the “Services”). Site Visitors, Application Users, and Subscribers. Client Users  are referred to collectively as “Users” or “you”, interchangeably.

When a Client User signs up for Services, we keep a record of medical information that a Client User provides to us or that a Providing User provides to us, as well as any medical information that Company employees or independently contracted Providing Users provide to Client Users via our platform (such as information on and related to your diagnosis). Medical information means any information that:

• identifies a Client User; and
• relates to a Client User’s past, present, or future physical or mental health, treatment, the provision of health care services or payment for treatment.

This includes medical history, diagnoses, treatments, current medical condition, and use of prescription medications. If you are a US Client User, your personal information in our possession is protected health information protected by the Health Insurance Portability and Accountability Act of 1996, as amended, and the applicable provisions of the Health Information Technology for Economic and Clinical Health Act.

In order to ensure the free, informed, and unambiguous consent of our Users to the collection and use of personal data, the Company requires all Users to read this Policy and accept the terms and conditions contained herein, together with consent to release personal data subject to such terms and conditions.

1. Data We Collect. The Company collects the following data on all Users (“User Data”):

• Personal identification information, such as name, email, address, and phone number.
• Information on your browser, IP address, location, and device.
• Information on your behavior on our website, including pages viewed, links clicked, keystrokes, and actions taken including copy/paste functions.
