About Malama
Malama is a community-centered maternal health organization providing tech-enabled care for pregnant and postpartum women. With a program reach of over 50,000 patients, Malama delivers doula services, Community Health Worker navigation, care coordination, prenatal and postpartum health education, and a CDC-approved Diabetes Prevention Program (DPP) designed for the postpartum population.
Our model is built on culturally congruent, trauma-informed care — meeting members where they are: in their homes, clinics, and communities. Malama's work has produced measurable results, including a 38% reduction in pre-term labor rates and a 9% reduction in C-section rates compared to gestational diabetes mellitus (GDM) population averages.
Position overview
The Malama Ambassador is a community-embedded birth equity advocate who serves as a trusted connector between high-risk pregnant and postpartum Medi-Cal members and the clinical, social, and peer support services they need.
Embedded in WIC clinics, Federally Qualified Health Centers (FQHCs), and community-based organizations (CBOs) across the NorCal region, the Ambassador performs outreach and eligibility screening, and attends key events on behalf of Malama.
This is a great entry point for recent graduates or early-career advocates who want to build a meaningful career in community health and are passionate about birth equity and reproductive justice.
Key responsibilities
- Community outreach & engagement
- Conduct in-person outreach at WIC clinics, FQHC waiting rooms, and CBO program sites across assigned counties
- Build trusted relationships with pregnant and postpartum Medi-Cal members, particularly those who are Black, Indigenous, or from communities of color
- Distribute multilingual outreach materials (Spanish, Tagalog, Vietnamese, and other languages per county demographics)
- Represent Malama at community events (baby showers, health fairs, partner-hosted convenings)
- Eligibility screening & referral
- Screen community members for program eligibility per CalAIM criteria
- Conduct warm handoffs to Malama's care coordination, doula, and CHW navigation teams
- Partner site integration
- Embed in partner site workflows at assigned WIC, FQHC, and CBO locations on a regular, scheduled basis
- Collaborate with partner site staff to co-develop and refine referral pathways
- Support MOU implementation and documentation of Ambassador activities at each partner site
- Documentation & reporting
- Document all member interactions, referrals, and outcomes in Malama's PointClickCare electronic health record (EHR) system
Qualifications
Required
- Current Community Health Worker (CHW) certification OR equivalent community health experience
- Deep ties to and lived experience in the communities served: Sacramento / San Bernardino / Fresno / Los Angeles / Alameda
- Commitment to birth equity, reproductive justice, and culturally congruent care
- Strong interpersonal skills and ability to build trust with diverse populations
- Ability to travel regularly across assigned counties for partner site visits
- Comfort with engaging with CBOs, following up, and tracking outreach activities
