Translation:

It happened in Przybyszew on the twenty-fifth of February, one thousand eight hundred ninety-seven at one o’clock in the afternoon.

There appeared Jan Sek, a farmer from Dziarnów, thirty years old, in the presence of Jan Nowicki, fifty-two years old, and Wawrzyniec Smolaga, sixty years old, both farmers from Dziarnów, and presented to us a female child, declaring that she was born in Dziarnów on the twenty-second of February of this year at seven o’clock in the morning,

to his lawful wife Józefa née Smolaga, twenty-four years old.

The child was baptized today and given the name Anna,

and her godparents were Adam Szolaga and Józefa Sęk.

This act was read aloud to the declarant and witnesses, who do not know how to write, and was signed by us.
