Work Completed

• Confirmed Gravity Forms is not active in the plugin list.
• Confirmed REST route /sal/v1/contact-intake is registered for POST.
• Confirmed leads 61 through 67 have Litify sent and email sent; Teams remains failed with HTTP 400.
• Repaired two dead Spanish page forms that had action="#", no submit handler, no backend field mapping, and no GCLID capture:
  • /abogado-lexington/, WordPress post ID 63356
  • /abogado-louisville/, WordPress post ID 63357
• Repaired the TRIMARC post form at /we-use-the-dot-database-to-find-crash-footage/, WordPress post ID 51560, by replacing the brittle static form section with [sal_contact_intake].
• Flushed WordPress object cache and Cloudflare helper cache after repairs.
• Created local report: /Users/samaguiar/Repos/litisail-vault/reports/form-fire-qa-2026-05-13.md.

QA Evidence

Final slow route matrix covered 15 discovered active form URLs. Each returned HTTP 200, rendered one visible form, included the live /sal/v1/contact-intake endpoint, had no dead action="#", and passed the no-send endpoint fire test. The lead table stayed at 67 before and 67 after, so no fake lead, email, Litify write, or Teams notification was created.

Routes checked: homepage, contact, Louisville Injury, both Spanish accident pages, passenger accident page, TRIMARC post, four injury sidebar pages, and four footer form posts.
