When a currently subscribed member takes a sign-off class, the instructor submits a Formbot. This form is automatically sent to the account management team.
Your sign offs are viewable on your member account on your member account profile. You access your member profile by clicking on the person icon on the member portal and viewing the tabs.
Periodically check to make sure you have all of your equipment and safety sign-offs. Email email@example.com if you are missing any sign-offs that you have earned. Include as much info as you can so we can validate (e.g., date, instructor, payment receipt, etc.).
A sign-off is a safety process, not a right. If we cannot validate a sign-off, then it will not be given.
