LEAM is a mobile companion app for people with periodic treatments. Its function is to help you record how you feel, prepare for your treatment sessions and generate personal summaries you can share with your doctor.

LEAM is not a medical device, does not diagnose, does not recommend treatments and does not replace the evaluation of a healthcare professional.

3. Permitted use

You may use LEAM to:

• Record your mood, energy, symptoms and personal notes
• Prepare for your treatment sessions
• Generate summaries of your personal record to share with your doctor
• Set optional reminders related to your treatment

4. Prohibited use

The following is prohibited:
