• Must include the honeypot company field but keep it visually hidden and unfocusable.
• Must preserve required fields: full name, phone, case type, injured status, consent. Email and message can remain optional unless Sam chooses otherwise.
• Must not introduce dead forms with action="#".

QA Expectations

Use Playwright for rendered QA across desktop and mobile. Verify form visibility, layout fit, no overlap, no clipped text, keyboard focus order, hidden tracking fields, endpoint attributes, and no console errors. Include screenshot paths or artifacts in the handoff.

Run no-send endpoint tests with the honeypot field populated. Confirm HTTP 200, success redirect, and no new row in wp_sal_contact_leads. Record the lead count before and after.

Known Context

Recent repair found dead Spanish page forms and a brittle TRIMARC static form. Those were fixed by moving to the maintained shortcode. Homepage still fires but lacks hidden gclid and page_title, and should remain approval-gated.
