• Found the earlier SAIL Knowledge Base handoff from 2026-05-28 stating the same removal was requested but not executed.
• Removed the live v4-offices row from the top contact/form section. This was the visible Louisville and Lexington office-card block after Injured? Let's Get Started.
• Removed the matching v4-offices and v4-office CSS, adjusted the desktop and mobile spacing, and preserved the intake form plus bottom CTA.
• WordPress REST update succeeded at modified time 2026-05-29T11:12:51, page stayed published.

QA Evidence

• Raw WordPress check after update: v4-offices=false, v4-office=false, target heading false, Available 24/7=false, #au-form-v4 still present, [sal_homepage_intake] still present.
• AU form CSS brace check passed: 42 opening braces, 42 closing braces.
• Public HTML checks on bare and cache-busted URLs showed no Our Physical Offices, Physical Offices, Available 24/7, or v4-offices.
