All incidents, injuries, and near misses must be reported immediately to the supervisor and recorded on the company’s Incident Report Form.
The supervisor must inform the Director (Blake Birkett) immediately within the same working day.
If there is a serious injury or illness, a death or a dangerous incident, you must report it to us immediately on 13 10 50 as an urgent investigation may be needed.

You must take care not to disturb the incident scene until an inspector arrives at the site, or until direction is given by an inspector. You can help an injured person and ensure safety of the site.

• An overview of what happened – the date, time and location of the incident. • Information about anyone who was injured, including their name, date of birth, contact details and relationship to you (for example: worker, volunteer, contractor, member of the public). • Information about the injury, treatment received and, if transported to hospital, the name of the hospital. • Your details including the legal and trading name, ABN, ACN and contact details. • Immediate action that was taken to make the site safe. • Further action that was taken, or will be taken, to prevent the incident happening again.
