Update: Notion Power Up service is no longer available. Instead of using the webhooks with the Google Form, I have created a table on Notion that people can add their requests to. Link to table here.

a1) Name to Give this Report/Data : (text box)

a2) Data Subject Area (marketing , product , user interaction patterns, other)

b) Data Subject Area Additional comments : ad hoc option comments requestor can add

c) Detail Level of Report (Detailed, Hourly, Daily, Weekly , Monthly, Quarterly, Yearly, Other)

c2) Additional Information about data in Report and how it is produced - such as top 10, etc

d) Frequency this Data/Report is Generated (On Demand, Daily, Weekly, Monthly, Quarterly, Yearly)

e) How Report/data is Accessed (sent to Slack channel, GA Dashboard, GS Dashboard (formed from Supermetrics data), saved into Notion area)

f) Report Access Details ( Slack Channel to send to, Notion area to write to, report name in Google Dashboard, report name in GS Dashboard)

g1) Report/Data Fields, their type, & Description to include (if this can be an extensible array of text entry boxes (10 rows presented but can add more) would be great. Each row would be for one report/data field and would have 3 text boxes.The first would be the name of the data field . The second would be if the value in the field is to be shown at its original detail level or summed up. It would be a drop down with values: Detail, Min, Max,Avg, etc), and then to the right a Notes box that allows requestor to add more optional info about how the field value is rendered

h) Prioritization (Low,Medium,High,SuperCritical)
