DecisionOps is a production-oriented Engineering Decision Governance System designed to solve a recurring failure mode in large software organizations:

In most teams, critical technical decisions are made across Slack threads, Jira tickets, pull requests, and meetings. The outcome is often recorded—if at all—as free-form documentation with no enforcement, no lifecycle awareness, and no systematic way to revisit decisions when assumptions change. As a result:
