Title: Fragmented health records across providers

Context: The current landscape of health records is highly fragmented across healthcare providers. This fragmentation stems from a lack of interoperability between electronic health record (EHR) systems, continued use of manual data entry or handwritten notes, and poor integration with patient engagement tools. As a result, healthcare providers often lack access to a patient's complete medical history during diagnosis and treatment.

This gap in information can lead to unnecessary repeated diagnostic procedures, increased costs for patients, and suboptimal or delayed care. Furthermore, fragmented and unstructured data increases the risk of medical errors, misinterpretation, and potentially life-threatening safety issues. The lack of seamless information flow also hinders patient education, coordinated care, and effective remote monitoring, ultimately compromising the overall quality of care.

Goals:

  1. Promote the modernization of healthcare information systems by improving interoperability and supporting a unified, accessible electronic health record system.
  2. Accelerate provider adoption and effective use of digital tools to enhance quality, safety, operational efficiency, and patient engagement in healthcare delivery.
