Discuss the specifics of augmenting the Oncology POC Via Form v1.1 to make it suitable for collecting the cervical cancer screening data indicators that satisfy the Ministry of Health/USAID reporting needs.

Resolutions from this meeting

It was resolved that the form should be amended as follows:

1. Rename it from Oncology POC VIA Form to Oncology POC Cervical Cancer Screening Form. This best signifies its broadened scope as it now goes beyond just VIA testing to include VILI tests, Pap smear and HPV tests as well.

2. Amend the Current visit type question in the Pre-Clinical Review section as follows:

   

   • Rename the Current visit type question to Current visit and remove the New option from its list of answers.
   • Add a new required question, Visit type with the following options as possible answers:
     • Initial screening
     • Routine screening
     • Post-Treatment screening
     • Treatment visit
     • ~~Post-Treatment~~ (option removed following discussions with Eve, Pet, Doris and Sally)
     • Complications

3. Rename the VIA Test page to Routine Screening and in addition, make the following changes:

   • Create a question named Screening method with the following options:

     • VIA or VIA/VILI
     • Pap smear
     • HPV

   • If VIA or VIA/VILI is chosen, display the VIA or VIA/VILI test result, Cervix impression, Vagina impression and Vulva impression questions. Remove the VIA test result question from the form:

     

     And replace it with a question named VIA or VIA/VILI test result with the following answers:

     • Negative
     • Positive
     • Suspicious for Cancer

     When Positive is selected, show a subsequent multi-select question named Observations noted from positive VIA or VIA/VILI test with the following options:

     • Abnormal vessels
     • Aceto-white
     • Excessive bleeding
     • Friable tissue
     • Punctuations
     • Ulcerations
     • Yellow staining lesion

   • If Pap Smear is chosen, display the following question:

     • Pap smear test result with the following options:
       • Normal
       • ASCUS/ASC-H
       • LSIL
       • HSIL/CIS
       • AGUS
       • Invasive Cancer

   • If HPV is chosen, display the following questions:

     • HPV test result with the following options:
       • Negative
       • Positive
       • Unknown
     • If Positive is chosen, display a question named HPV Type with the following options:
       • PCR HPV Type 16
       • PCR HPV Type 18
       • HPV Type 45

4. On the Procedure Done page, remove the following options from the Procedure done this visit question:

   

   • Pap Smear
   • Cryotherapy
   • LEEP

   Rationale: Pap smear is a screening option whereas Cryotherapy and LEEP are treatment options.

   The options listed above will be moved to the Treatment Plan page of the form as options to the Treatment method question under the Pre-Cancer Treatment section. In addition, it was proposed that the following options should be added to the list of answers:

   • Endometrial biopsy
   • Punch biopsy

5. Add a Treatment Plan page with a section named Pre-Cancer Treatment. This section should have the following questions:

   • Treatment method

     Which has the following options:

     • Cryotherapy
     • LEEP
     • Thermocoagulation

   • Status of cryotherapy - displayed only when Cryotherapy is selected as the treatment option. Answers to this question include:

     • Done
     • SVA
     • Postponed
     • Referred

   • Status of LEEP - displayed only when LEEP is selected as the treatment option. Answers to this question include:

     • SVA
     • Physical
     • Referred for LEEP

   • Status of thermocoagulation - displayed only when Thermocoagulation is selected as the treatment option. Answers to this question include:

     • Done
     • SVA
     • Physical
     • Referred

6. Rename the Management Plan page to Follow-up Plan and move it so it sits just before the Next Appointment page.

Progress made on implementing these changes

• Collate the changes that need to be made to the form and its constituent components in a GitHub repository https://github.com/denniskigen/via-form-review. (DONE)
• Sit down with Pet where we review the changes in the repository with relation to the AMRS and OCL concept dictionaries. It is at this stage where Pet helps to create new concepts as needed. (DONE)
