It was resolved that the form should be amended as follows:
Rename it from Oncology POC VIA Form to Oncology POC Cervical Cancer Screening Form. This best signifies its broadened scope as it now goes beyond just VIA testing to include VILI tests, Pap smear and HPV tests as well.
