Causes of Hypertension & Hypokalemia

HTN + HYPOkalemia

1st step is to check Renin and aldosterone.

If BOTH are ELEVATED:

Secondary hyperaldosteronism

• Renovascular hypertension
• Malignant hypertension
• Renin-secreting tumor or reninoma - benign juxtaglomerular cell neoplsms
• Diuretic use

If Aldosterone is elevated and Renin is low/suppressed:

Primary hyperaldosteronism (Conn's syndrome)

• Aldosterone-producing tumor
• Bilateral adrenal hyperplasia or adrenal adenoma

If BOTH renin & aldosterone are LOW:

Non-aldosterone causes:

• Congenital adrenal hyperplasia
• Deoxycorticosterone-producing adrenal tumor
• Cushing syndrome
• Exogenous mineralocorticoids
