TOF occurs due to anterior and cephalad deviation of the infundibular septum during embryologic development, resulting in a malaligned VSD with an overriding aorta.

As a result, the patient has RVOT -> systolic murmur

Squatting increase the PVR (afterload) and decrease the degree of right-to-left shunting across the VSD
