BSA Medical Form

Use the BSA Medical Form, and fill out all three parts (Part A, B, and C) for all persons staying more than seventy-two hours. Part C is the physical exam which is required for anyone staying seventy-two hours or longer.

Please note you must use the Part C medical form and not a sports physical, school physical, or other form. The Part C must also be signed by either an MD, DO, physician assistant, or nurse practitioner. Registered nurses, chiropractors, physical therapist do not qualify.

Persons staying less than seventy-two hours need to use the same form, but Part C (physical exam) is not required.

Medications

Medications are stored in a locked container in your campsite provided by the camp and be administered by an adult leader in your troop. We will provide the container and lock. Please use these instead of your own lock boxes for all medications to enable us to evacuate camp quickly if need be, and so medical staff can access the box in an emergency.

When administering medication, please record the medication name, dosage, frequency, etc. in a log that camp will provide.

Medications requiring refrigeration may be stored in the health lodge refrigerator.

All medications need to be in their original pharmacy container including the label with the correct person’s name, dosage information, and pharmacy and physician information.

Troops that are not comfortable storing and administering medications may have them stored in the health lodge. An adult from the troop needs to be present whenever Scouts receive medications.

Any emergency-use medication such as inhalers and epi-pens may be stored on the person who requires it, and should be with them at all times during their stay.
