The goal of this document is to overview our feedback processes and clearly define the steps included. Standardizing this will allow us to move through it efficiently, provide consistency across the team, and afford the ability to improve it over time.
Roles
Everyone at the company has a important role to play in this process. Each role is distinct, but requires full participation from the others in order to be successful.
- Employees: Complete feedback forms and come to review processes with thoughtful notes and ideas related to their performance.
- Leadership Team: Set aside time to carefully evaluate performance and arrive at meetings with structured feedback.
- Evaluation Committee: Ensure equity across the organization and support employees in their vocational training.
Feedback Forms
There are two types of feedback we collect. The objective is to keep questions consistent so that the data can be evaluated over time.

Review process
There are three types of review processes that are scheduled on a quarterly and semi annual basis. These require involvement of the leadership team and evaluation committee.
Quarterly Reviews
These happen during the first month of a new quarter.
Pulse check (Semi-annual)
These happen at the end of Q1 (April) and Q3 (October).
- Info gathering (1 week)
- [ ] Quarterly Review Form sent out to entire company (self and upward)
- [ ] Manager Feedback Form sent out and completed for each direct report (manager)
- Info comparing (1 week)
- [ ] Evaluation committee reviews all completed forms
- [ ] Relevant Quarterly Review Form feedback is shared with managers
- Info sharing (1 week)
- [ ] Manager and direct reports meet to go over form feedback
Deep dive (Semi-annual)
These happen at the end of Q2 (July) and Q4 (January).
- Info gathering (1 week)
- [ ] Quarterly Review Form sent out to entire company (self and upward)
- [ ] Manager Feedback Form sent out and completed for each direct report (manager)
- Info comparing (2 weeks)
- [ ] Evaluation committee reviews all forms/recommendations and makes decisions on promotions/raises (Make sure we're keeping the team in lockstep across salary bands/level structure/etc.)
- [ ] Evaluation committee shares Quarterly Review Form feedback with managers
- [ ] Managers complete written Notion reviews for each employee
- Info sharing (1 week)
- [ ] Manager and direct reports meet to go over feedback
- [ ] (When applicable) manager delivers promotion/raise update
