Monteggia's fracture

of proximal 1/3 of ulnar, along with dislocation or instability of the head of the radius

S/S

Pain+ swelling to elbow , reduced ROM of elbow; radial head may be palpable

Cx

Posterior interosseous nerve or radial nerve may be affected. These nerves supply the wrist and finger extensors

Tx

In pediatrics (skeletally immature), closed reduction and casting is enough

ORIF is usually necessary for adults (remember, once # is open, do ORIF regardless of age)

Galeazzi fracture

of distal 1/3 of radius, along with dislocation or instability of the distal radio-ulnar joint

S/S

Pain+swelling with or without deformity over wrist

Tx

Colles Fracture

of the distal radius (within one inch of the distal radio-ulnar joint) with dorsal deviation of the distal piece

caused by fall on outstretched hand with wrist extension

classic "dinner fork" deformity may be seen
