The current gap came from workflow drift: visible hero/body images and WordPress featured-image metadata are separate surfaces. Several pages had images in their content or custom hero, but _thumbnail_id / featured_media was not assigned. Five of the six pages had been modified between 2026-05-16 and 2026-05-18, so this was tied to recent page/template work and old exclusion assumptions, not the old March incident.
