What is broken on the live DoorDash page

Confirmed defects (all five):
- No On This Page TOC dropdown. Standard sa-toc-wrap details element, positioned above the snippet, is missing.
- Snippet uses dated design. Standard is left-only orange border (border-left: 4px solid #f89c22) with no top/right/bottom border.
- Sidebar Call Now button alignment is off inside the Get Back To Living Your Best Life CTA panel.
- FAQ design is plain bold-question plus paragraph-answer. Standard is the sa-faq-item details/summary accordion.
- Two banned FAQs present: How long do I have to file a claim (statute of limitations, cites KRS 304.39-230) and What if I was partly at fault (comparative fault, cites KRS 411.182). Both are banned per website-rules-new section 12B everywhere except the SB 195 page itself.
Why this slipped through (root cause)
Three audits that were supposed to catch this and what happened to each
- SOL FAQ Sitewide Audit + Batch Replacement Plan (2026-04-05). Scanned all 411 existing pages. Identified 149 with SOL FAQs. Sam corrected all 149 answers. Deployment plan was built. Deployment phase was NOT STARTED per the Notion handoff. Even if it had deployed, this audit was a REPLACE not a REMOVE pass, and DoorDash was not in the 411-page universe yet.
- SB 195 Handoff (2026-05-01) locked in section 12B and section 12C rules. Sam said verbatim: no comparative fault or statutes of limitation go on anything else as FAQ or fluff. Rule was locked into website-rules-new v3.4 section 12B and section 12C. Skill pre-publish-qa upgraded to v3.7 with FAIL-level scans for both. The sitewide enforcement scan locate-violations.py was QUEUED, NOT YET RUN. The SB 195 page rewrite was the blocker, and the queue never cleared.
- Sitewide Compliance Enforcement (2026-04-01 to 04-03). Removed 51 standalone statute of limitations and comparative fault SECTIONS. Targeted standalone H2/H3 sections, not FAQ accordion entries.
What the recurring QA actually checks (and what it misses)
Sunday Site QA ran today (2026-05-17) across 528 pages, found 275 with findings. By issue type the recurring scanner only enforces title_h1_mismatch and missing_finished_links. It does NOT scan for section 12B banned FAQ topics, section 12C leading-article headers, snippet border compliance, missing sa-toc-wrap, or sidebar CTA layout.
Sitewide Brand Drift Sweep (2026-05-12) scanned 542 URLs for CSS and copy patterns (orange-bg-fill, banned-deserve, deprecated classes) but did NOT scan FAQ topic content.
DoorDash-specific touch history
The DoorDash page was created 2026-04-20/21 as Hub A Spoke A2 (after the April 5 audit had already snapshotted the universe). It was then touched four more times:
