Importance: High. The account-wide GitHub Actions CI block is roughly 11 days old and is the top open item. Routine tasks: mark pending (QA items open).

What was done

• Canonical repo-health-runner.sh invoked first and completed in full (exit 0, all 11 repos) inside the sandbox. This contradicts the carried "runner cannot finish in sandbox" claim, now downgraded (QA P9).
• All 11 repos scanned. Watchdog: report fresh, no miss logged.
• 3 autonomous fixes (spec's always-allowed list): removed 6 stale 0-byte index.lock files left by the runner's own git operations; hardened .gitignore in sail-cases (dirty 9 to 1) and sail-hr (dirty 19 to 4), ignoring .claude/skills/ agent-skill copies plus PII paths (case-files/, teams-*.csv, employee-monitoring-*).
• Rollback snapshots and note: ~/Documents/Codex/repo-health/backups/2026-05-23/.

State of the repos

• Zero commits and zero CI runs across all 11 repos in the 24h window. No default-branch HEAD moved.
• 4 workflows disabled_manually (1 genuinely dead, 3 parked for billing).
• Dependabot: 0 of 10 git-backed repos. The 2026-05-17 staged bundle is still unapplied (6 days old).
• Secrets clean. NOTION_TOKEN is present on Notion-wiki, so the 8-day-outage cause is not recurring.
• Large dirty trees persist: sail-marketing 2615, sail-litify 326 (the latter on a Codex-agent-owned branch).

Why the session ended

Completed normally. Scan, report, autonomous fixes, and all output writes finished. Nothing blocked.

QA items pending Sam's approval

Full A/B/C/D for each is in the run report and the Codex QA queue. Summary of the recommended option:

• P1 Add a NOTION_TOKEN alias to the vault (ends a daily V4 false-positive).
• P2 Delete the dead sail-seo scope-scan.yml once main is unblocked; re-enable the 3 parked workflows when P7 clears.
• P3 Hold the 2 big dirty trees for Sam / the owning Codex agent. No blind bulk commit.
