The text input fields are not a realistic length. They should be sized according to the content that will go into them. In this case, they should not be 1 input per line, if possible. If we can only have 1 input field per line then please make the fields max-width 360px

Please give the Create Patient button a hover state with colour IBM Teal-70 or #005D5D

Could you please put an empty div beneath the last section of the form so that the form can be scrolled more into the centre of the screen. At the moment, when the bottom of the 3rd section is approximately 16px above my viewport, i can't scroll the page any further.

The label on this field should probably just Post code
