A patient who is already established at Klinic, but is starting a new service line (e.g., currently in SUD treatment(s) but now beginning Psychiatry & Mental Health treatment).

Exception: SUD/MAT patients adding General Psychiatry
An existing SUD/MAT patient starting general psychiatric care does not need the full Psychiatric Clinical Intake Form. Book the Psychiatric Medication Management (DD) 30 appointment instead of a New-To-Service initial visit. The provider already has enough history from the SUD intake and charting notes to complete the visit.
This exception applies to the intake questionnaire only. Standard automatic forms for the appointment, like the confirmation email and reminder email, still go out as usual. If a patient has not completed them by the time of the visit, the provider can cover that ground live during the appointment.
