Clean up Sam Hub Treatment QA reporting so Sam can glance at high-value issue signals, then define the next build for AI and agentic capabilities inside Sam Hub.

Sam Hub Treatment QA issue-first cleanup
- Reduced the Treatment QA panel to issue/value tiles rather than raw volume totals.
- Removed inline long lists from the Sam Hub payload.
- Removed active medical request, records-request, calls-missing-summary, and calls-missing-length visualizations.
- Kept source reports behind the remaining tiles.
- Updated report columns for stale treatment updates, employment missing, primary provider missing, and treatment damages.

- Removed provider/source-match display noise across the Treatment QA report set.
