Clean up Sam Hub treatment QA so the first glance shows only useful issue/value signals, with long source lists behind tile-linked Salesforce reports.

• Deployed Sam Hub controller, test, LWC, and six report updates in live Litify deploy 0AfUV000001Z2kj0AC.
• Reduced Treatment QA to 8 tiles: Treatment Damages $50K+, Treatment Damages $100K+, Open Treatment Damages, Stale Treatment Updates, Exacta/Aptiva Needs Drive Check, Employment Status Missing, Primary Provider Missing, and Calls Missing QA Score.
• Removed inline long source sections from the hub payload. Live readback confirmed 8 tiles and 0 source sections.
• Removed active medical/request volume, calls missing summary, and calls missing length from Sam Hub.
• Deleted obsolete reports in live deploy 0AfUV000001Z2nx0AC: Sam Hub - Records Requests 21+ Days, Sam Hub - Calls Missing Summary, and Sam Hub - Calls Missing Length.
• Updated Stale Treatment Updates, Employment Status Missing, and Primary Provider Missing reports to group by attorney and include incident date, days since incident, rating, rating explanation, damages, minimum known coverage, and injuries.
• Updated treatment damages reports to include incident date, days since incident, rating explanation, damages, minimum known coverage, and injuries, with no request-count or source-match columns.
• Updated the LWC money formatting so actual money values display as currency while the $50K+ and $100K+ tiles remain case counts.
