    1. 상담 및 예약 신청하는 시술, 수술 종류
    2. 상담 및 예약 신청한 의료기관으로부터 수집하는 정보: 내원여부, 상담여부, 시술여부, 시술 예정일, 시술 가격
  3. 민감정보의 보유 및 이용 기간
    1. 당사는 개인정보 수집·이용 목적이 달성된 후 해당 정보를 파기합니다. 다만, 민원 처리, 분쟁해결, 법령상 의무 이행을 위해 관련 법령에서 보관을 요구하는 계약, 결제, 서비스 제공 내역 등은 법령이 정한 보존 기간(5년)에 따라 보관합니다
    2. 보유기간 내라도 이용자 또는 법원, 정부 기관에서 해당 개인정보를 조기에 삭제 또는 파기하도록 요청할 경우 그에 따릅니다
  4. 동의를 거부할 권리 및 거부하는 경우의 불이익
    1. 이용자는 이 개인정보 수집 및 이용 동의를 거부할 수 있습니다. 이 동의는 의료기관 상담 및 에약 신청 서비스 제공에 필수적이므로 위 사항에 동의하셔야 병원상담신청이 가능합니다.

    1. To provide medical institution consultation application and reservation services
  2. Items of Sensitive Information Collected
    1. Types of procedures or surgeries for which consultation and reservation are requested
    2. Information collected from the medical institution to which the consultation and reservation are requested: visit status, consultation status, procedure status, scheduled procedure date, procedure price
  3. Retention and Use Period of Sensitive Information
    1. We destroy the information once the purpose of collecting and using the personal information has been achieved. However, records that must be retained pursuant to relevant laws—such as contracts, payments, and service provision details required for handling civil complaints, dispute resolution, or fulfilling legal obligations—are preserved for the statutory retention period (5 years).
    2. Even within the retention period, if the user or a court or government agency requests early deletion or destruction of the relevant personal information, we will comply with such request.
  4. Right to Refuse Consent and Disadvantages in Case of Refusal
    1. The user may refuse to consent to this collection and use of personal information. However, this consent is essential for providing the medical institution consultation and reservation application service; therefore, hospital consultation applications cannot be submitted without agreeing to the above.
